Emerald's Patient Advocates look at the needs of patients and their families based on their own individual circumstances and provide assistance, information on local services or make referrals as appropriate. For elders the focus is on helping those who wish to remain in their homes do so, independently and safely as possible. Common concerns can include, but are not limited to: Hospice, bereavement, medical-legal issues, disabilities, home care, caregiver issues, patient and family education, patient assistance programs, dementia and support.

	“Do we have a copy of your health care proxy for your medical record?” A health care proxy is a legal document that allows your chosen agent to make health care decisions in the event that you may be incapable of making such decisions. If you do not have a health care proxy, please contact our Staff and/or the Front Desks or contact our Patient Advocate Department and we will provide one for you free of charge.
